Our goal at the LRHS clinic is to provide the best possible care for your child in the event of illness or injury while at school and to promote good health and prevention of disease. We have outlined areas where additional documentation is required for the safe management of students with health conditions/concerns.
Students who have a history of severe allergic reactions and asthma attacks may self-medicate in school if they have a current written prescription from their physician stating they may self-medicate and carry the medication on their person. The two forms necessary for students to carry their own medication on campus are:
1) Medication Authorization Form
2) MCSHS Contract to Carry and Self-administer form
Students who have a history of diabetes may also carry their own supplies and self medicate. Students with diabetes have additional options for the management of their condition consistent with their Physician's orders. Please use the following forms if your student requires diabetic supplies while on campus:
1) Diabetic Medical Management Plan
2) MCSHS Contract to Carry and Self-administer form
Many conditions can be treated at school with over-the-counter (OTC) medications. With proper authorization, documentation and signatures, arrangements can be made for over-the-counter medication to be given to students. This may prevent loss of time out of school. To have Prescription or OTC medication administered in school:
Complete the Medication Authorization Form.
- Parent/guardian brings the Prescription/OTC medication to school clinic.
- Clinic staff will process order and verify medication amount with parent.
- Unused medications will be returned or destroyed per policy.
Students requiring medication or medical treatment while on a school field trip require a completed Medication Authorization Form for the specific prescription/OTC medication required. Field trip chaperones will be made aware and trained on the safe administration of medications to specific students per the Medication Authorization Form and District policies.
All parents/guardians must keep the information on their student’s Annual Student Medical/Health Information Form updated throughout the school year as needed. Please download and complete this form (see link on this page) if there are ANY changes to your student's health conditions or contact information.
COVID-19 Information:
The clinic staff has developed procedures consistent with CDC and Florida Health Dept. guidelines to protect students and staff during the covid-19 emergency.
It is CRITICAL for parents to keep their student out of school if they complain of a new onset of:
- Temperature of 100.4 or greater
- Sore Throat
- NEW uncontrolled cough that causes difficulty breathing (for students with chronic allergic/asthmatic cough, a change in their usual cough)
- Diarrhea, vomiting, or abdominal pain; or
- New onset of severe headache
Students who report to the clinic with ANY of the above symptoms will be isolated and sent home immediately.
